When ordering processes go wrong.

I recently experienced the following as a customer:
I made an enquiry about a product and was quoted a price.
I placed the order and was promised delivery.
Delivery didn't happen.
I sent a reminder about the order, and was promised delivery again.
Delivery didn't happen, again.
I made a final order request, and delivery happened, but the price was double what I was originally quoted.
How well is your order process working in the real world?
When was the last time you tested it from the customer's perspective?
As a business owner, you should be doing this at least twice a year.
